You hereby acknowledge that fitness classes are not without inherent risks, and that participation in such classes may result in serious injury, including but not limited to sprained joints, broken bones, and paralysis, or death. Your participation is conditioned on the following release of liability:

In agreeance of my participation in Fitness classes with Anita..aka: Coachabc YOU, ON YOUR OWN BEHALF, KNOWINGLY, VOLUNTARILY, AND FREELY ACCEPT AND ASSUME ANY AND ALL RISKS, WAIVER OF ALL CLAIMS. BY PARTICIPATING, YOU EXPRESSLY RELEASE AND WAIVE ANY AND ALL CLAIMS, SUITS OR DEMANDS FOR PERSONAL INJURY, DEATH, PROPERTY DAMAGE OR OTHER LOSS AGAINST THE RELEASED PARTIES. TO THE FULLEST EXTENT PERMITTED BY LAW, THIS WAIVER IS INTENDED TO BE A COMPLETE RELEASE OF THE RELEASED PARTIES (ANITA, AKA: COACHABC, & ANY LOCATION SITE HOSTING FITNESS CLASSES with ANITA, AKA: COACHABC.) FOR ANY AND ALL NEGLIGENT ACTS OR OMISSIONS, AND ALL RESPONSIBILITY FOR PERSONAL INJURIES, DEATH, OR PROPERTY DAMAGE SUSTAINED BY ME OR ANY OF THE PERSONS ENGAGING IN FITNESS CLASSES WITH ANITA: AKA: COACHABC, WHETHER ARISING OUT OF OR RESULTING FROM MY OR YOUR PARTICIPATION IN ACTIVITIES AND EXERCISES, OR NOT. YOU FURTHER AGREE TO DEFEND, INDEMNIFY AND HOLD HARMLESS THE RELEASED PARTIES, AND EACH OF THEM, AGAINST ANY AND ALL CLAIMS, LAWSUITS OR DEMANDS RESULTING FROM ANY LOSS, INJURY, DAMAGE, OR DEATH, AS WELL AS PROPERTY DAMAGE, AND ATTORNYES FEES ARISING OUT OF, CONNECTED TO, OR RELATING IN ANY WAY TO THE PARTICIPATION IN FITNESS CLASSES WITH ANITA, AKA: COACHABC.

I hereby release, waive, discharge and agree to harmless consideration towards ANITA: AKA: COACHABC A Certified and Licensed instructor, as well as the location of any facilities, or owner of any facility used by ANITA, AKA: COACHABC as a result of my voluntary participation in FITNESS CLASSES WITH ANITA: AKA: COACHABC.

I have read and fully understand the above waiver and release from liability all of the afore mentioned parties IS EFFECTIVE FOR 180 days (6 months)
